Advantage Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team. We are committed to providing exceptional end-of-life care with dignity, respect, and compassion.

 

Job Purpose
The LPN provides skilled nursing care to terminally ill patients under the direction and supervision of a Registered Nurse and in accordance with physician orders and the interdisciplinary group (IDG) plan of care. All services are delivered in alignment with Advantage Hospice policies and standards.

Responsibilities

Essential Duties and Responsibilities

  • Understand and adhere to established policies and procedures.

  • Implement the IDG nursing care plan for each patient.

  • Provide nursing services, treatments, and preventive procedures as assigned.

  • Observe and report changes in patient condition to the RN and physician.

  • Teach and counsel patients and families regarding care needs.

  • Evaluate, with the RN, the effectiveness of nursing services provided.

  • Maintain accurate and complete records of patient care.

  • Participate in medical record audits, staff meetings, IDG conferences, and in-services.

  • Take on-call duty, including nights, weekends, and holidays as assigned.

  • Submit schedule changes to the Clinical Director daily.

  • Assist the physician and RN in performing specialized procedures.

  • Prepare clinical and progress notes, as well as equipment and materials for treatments.

  • Observe aseptic technique as required.

  • Support patients and families in learning appropriate self-care techniques.

Qualifications

Qualifications

  • Graduate of a state-approved school of practical (vocational) nursing.

  • Current LPN license in the state of practice.

  • Minimum of one year of nursing experience preferred.

  • Acceptance of the philosophy and goals of hospice care.

  • Ability to exercise initiative and independent judgment.

  • Completion of a hospice training program prior to providing care.

Physical Requirements

  • Ability to participate in physical activity and stand for extended periods.

  • Ability to lift and perform regular bending and standing.

  • Capable of heavy lifting as needed for patient care.
