Hospice Social Worker Part Time

Advantage Hospice is seeking a dedicated and empathetic Hospice Social Worker to join our interdisciplinary team and provide vital emotional, spiritual, and psychosocial support. Job Purpose:

To deliver compassionate, holistic social work services to hospice patients and their families—providing emotional guidance, spiritual support, crisis counseling, and advocacy, while connecting them to essential community resources.

Responsibilities

What You'll Do

 

  • Perform initial and ongoing psychosocial, spiritual, and emotional assessments.
  • Collaborate in developing patient-centered care plans.
  • Provide individual and family counseling, including crisis intervention.
  • Serve as a bereavement team member, offering grief support as needed.
  • Act as an advocate for patients and families in IDG meetings.
  • Maintain accurate and timely documentation of all services and contacts.
  • Develop community relationships to enhance referrals and outreach.
  • Participate in staff meetings and continued professional development.

Qualifications

About You

 

  • MSW from a CSWE-accredited school and one year of health care experience.
  • BSW from a CSWE-accredited school + one year of experience in a health care setting (supervised by an MSW unless exempt).
  • Bachelor’s in psychology, sociology, or related field + one year of health care social work experience (with MSW supervision).

Physical Requirements:

  • Ability to sit, stand, bend, and lift light loads regularly.
