What You’ll Do:

• Conduct patient assessments and create individualized care plans
• Deliver top-quality clinical care and monitor progress
• Collaborate with LPNs, therapists, and home health aides to ensure holistic care
• Educate patients and families on health conditions and care techniques
• Provide compassionate, patient-centered nursing in a home health setting

What You Bring:

• Active RN license in Pennsylvania
• Minimum of one year of nursing experience (home health preferred, not required)
• A commitment to quality, compassion, and clinical excellence
• Strong communication and critical thinking skills
• Ability to work independently with confidence—and the full support of your team
