Are you passionate about making a difference in the lives of patients and their families during life’s most vulnerable moments? Advantage Hospice is seeking a dedicated and empathetic Hospice Social Worker to join our interdisciplinary team and provide vital emotional, spiritual, and psychosocial support.

Job Purpose:

To deliver compassionate, holistic social work services to hospice patients and their families—providing emotional guidance, spiritual support, crisis counseling, and advocacy, while connecting them to essential community resources.

Key Responsibilities:

• Perform initial and ongoing psychosocial, spiritual, and emotional assessments.
• Collaborate in developing patient-centered care plans.
• Provide individual and family counseling, including crisis intervention.
• Serve as a bereavement team member, offering grief support as needed.
• Act as an advocate for patients and families in IDG meetings.
• Maintain accurate and timely documentation of all services and contacts.
• Develop community relationships to enhance referrals and outreach.
• Participate in staff meetings and continued professional development.

Qualifications:

You may qualify for this role with one of the following:

• MSW from a CSWE-accredited school and one year of health care experience.
• BSW from a CSWE-accredited school + one year of experience in a health care setting (supervised by an MSW unless exempt).
• Bachelor’s in psychology, sociology, or related field + one year of health care social work experience (with MSW supervision).
