What You’ll Do:

• Assist in creating customized treatment plans for patients under the direction of a licensed physical therapist.

• Provide one-on-one care to patients in their homes, ensuring they progress toward independent, fulfilling lives.

• Collaborate with interdisciplinary teams to improve patient outcomes.

• Ensure quality rehabilitation services that meet federal, state, and local standards, as we continue to set the bar for excellence in home health and rehab.
