Nurse Practitioner (CRNP)

Advantage Hospice is seeking a compassionate and skilled Nurse Practitioner (NP) to join our dedicated team. As a Nurse Practitioner, you will work closely with the Hospice Medical Director to deliver advanced, compassionate care to patients and their families during the end-of-life journey. This role requires clinical expertise, independent decision-making, and the ability to collaborate effectively with an interdisciplinary team to provide the highest quality of hospice care.

Key Responsibilities:

• Conduct in-person assessments to monitor patient conditions and determine eligibility for hospice recertification.
• Work closely with the Medical Director and interdisciplinary team to provide personalized, patient-centered care.
• Ensure adherence to hospice admission and re-certification criteria to maintain appropriate levels of care.
• Communicate clinical findings to insurance case managers when necessary.
• Discuss advance directives, goals of care, and hospice services with patients and their families.
• Advocate for patient autonomy and dignity throughout their end-of-life care.

Qualifications:

• Master's degree in Nursing from an accredited Nurse Practitioner program.
• Current PA state licensure as a Nurse Practitioner (CRNP).
• National certification in Adult Health, Gerontology, or Palliative Care (ANCC, AANP, ONCC, NBCHPN).
• Hospice experience preferred, though experience in home care, acute care, or skilled nursing will be considered.
• Certification in Generalist Hospice and Palliative Nursing (CHPN) preferred.
• Advanced Certification in Hospice and Palliative Care Nursing (ACHPN) preferred.
